REQUIREMENTS SUBMISSION
REMINDER: PLEASE ENSURE THAT YOU HAVE CLICKED THE "APPLY TO COURSE" BUTTON AND HAVE YOUR REQUIREMENTS READY BEFORE ENTERING THIS ACTIVITY.
Kindly ensure to submit all requirements listed below:
- Copy of Valid PRC ID
- Copy of License to Operate issued for 2024
- Copy of Certificate of Inclusion of Laboratory issued for 2024
- Letter of Endorsement from the Pathologist or Head of the Agency certifying that the applicant is a practicing Medical Technologist in the laboratory.

- If your training will have a face-to-face session, do not forget to bring the original copy of your proof of payment for submission.
